Job summary

Elite Home Health Care seeks a Nurse Manager to lead clinical operations for our LHCSA in New York. You will oversee staff performance, orientation, evaluations, and ongoing training for Licensed Caregivers and RNs, ensuring high-quality, compliant care aligned with NYSDOH regulations.

Qualifications

• Graduate of an accredited nursing program with a Diploma/Associate/Bachelor/Master's in nursing.
• Active and unrestricted Registered Nurse (RN) license in New York State.
• Valid driver’s license with reliable transportation and current auto insurance coverage.
• Ability to travel locally for home visits, staff supervision, and emergency coverage.
• Demonstrated compassion, empathy, and leadership.

Responsibilities

• Monitor and evaluate clinical staff performance and ensure adherence to Elite Home Health Care’s standards of excellence.
• Provide coaching, mentoring and training for staff, including performance redirection when necessary.
• Conduct orientation and annual performance reviews of caregiver and nursing staff under NYSDOH regulations.
• Develop and review Personal Care Plans across all contracts and payor sources.
• Support recruitment, hiring and retention of home care staff to maintain service continuity.
• Monitor and operationalize change in condition protocols, including timely follow-up and reporting.
• Manage triage calls and post-hospitalization follow-ups with timely incident reporting.
• Develop emergency staffing plans for weather events or service disruptions.
• Participate in QAPI investigations, corrective actions, and follow-up.
• Collaborate on care planning with the Director of Nursing, physicians and team members.
• Schedule and oversee initial and recertification nursing visits in line with Medicaid contracts.
• Deliver onboarding and process updates for new nurses.
• Develop orientation programs and ensure staff training on NYSDOH standards and Elite policies.
• Monitor field visit completions and assessments for compliance.
• Perform other duties to support agency operations.
