Job summary

PeopleOne Health is seeking a Family Nurse Practitioner to provide primary and preventive care to patients aged 2+. The role involves autonomous practice with collaboration to improve health outcomes, including prescribing medications, ordering tests, and coordinating care across an interdisciplinary team.

The FNP will perform comprehensive health assessments, develop individualized treatment plans, and educate patients and families on wellness and disease prevention.

Qualifications

• Master's or Doctorate in Nursing required.
• Current CRNP license required.
• National certification required.
• DEA registration required.
• BLS and ACLS certifications required.
• 2-5 years NP experience in outpatient/primary care preferred.

Responsibilities

• Conducts comprehensive health assessments and exams.
• Diagnoses and manages acute and chronic illnesses with individualized treatment plans.
• Prescribes medications and non-pharmacologic therapies per guidelines.
• Orders, interprets, and evaluates diagnostic tests (labs, imaging, EKGs).
• Provides health counseling, disease prevention strategies, and wellness education.
• Performs minor medical procedures within scope of practice.
• Collaborates with specialists for referrals and continuity of care.
• Documents accurately in EHR per healthcare regulations.
