Summary

In collaboration with patients/families, case managers, physicians and the interdisciplinary team, the Lead Social Worker provides direct psychosocial and practical care including; social work assessments, problem identification, goal setting behavioral health intervention and implementation of care plans. The Lead Social Worker supports continual process improvement in the delivery of clinical social work intervention and care coordination to benefit patient care/outcomes throughout the organization.

Essential Functions

  • Serves as a clinical resource expert in a specialty area; provides administrative leadership in the absence of senior leadership and functions as a mentor for others.
  • May engage in academic work including research, grants, publications and education.
  • Assists Director of Social Work in administrative duties.
  • Acts as a resource for more junior Social Work staff and may provide input on performance evaluations.
  • May be required to provide coverage during absences and vacancies.
  • Provides clinical supervision to assigned departmental staff according to COMAR regulations in both group and/or individual settings; may serve as a field instructor for MSW candidates.
  • Provides case consultation to clinical staff for complex psychosocial problems.
  • Collaborates with senior management to assist with policy review, program development, and onboarding of new staff; implements and supports departmental objectives, staffing patterns and performance standards.
  • Completes full psychosocial assessment and plan of care for all patients in assignment area with positive indicators for social work consultation; provides direct psychosocial care and psychosocial transition planning for a full case load of patients in assigned area.
  • Works in a collaborative practice model, facilitating care coordination with the multidisciplinary treatment team for care progression to optimize patient outcomes.
  • Adheres to departmental and regulatory documentation standards and formulates clear descriptions of the problems, goals and interventions. Adheres to timely completion of medical record documentation.
  • Assists with resource referral, capacity determination, legal guardianships, adoptions, psychiatric referral, assessment of abuse/ neglect /IPV and charity care; identifies the need for and facilitates family meetings.
  • Leads or participates in interdisciplinary rounds if applicable for assignment area.
  • Participates in quality improvement activities; serves on committees within the medical center and community.
  • Assists with on-call coverage as needed.
  • Other duties as assigned.

In Addition

Community Psychiatry Program (CPP) Lead Social Worker - JHBMC

  • He / She collaborates with all disciplines to optimize patient outcomes; scope of services may include, but is not limited to: developing action plans to mitigate psycho-social barriers to discharge; patient/family engagement and education; employing interventions (crisis intervention, grief counseling, suspected abuse reporting) to address immediate needs.
  • Assists supervisors/managers with administrative duties.
  • Completes diagnostic intake in accordance with joint commission requirements.
  • Completes full psychosocial assessment and plan of care for patients and families who require intervention.
  • Provides direct psychotherapy and psychosocial care for a full case load of patients in assigned area
  • Monitors effectiveness of medication and observes for and reports side-effects of medication. Notifies and consults with physician if medication changes are indicated.
  • Complies with Joint Commission requirement to utilize Feedback Informed Care (FIT) with patients

Qualifications

Education:

  • MSW Required

Knowledge, Skills, And Abilities

  • Possesses subject matter and system expertise, enabling the individual to provide guidance to staff and counsel to leadership.
  • Knowledge of relevant community resources commensurate with illness, symptoms, mental status and impairments in functioning.
  • Knowledge of the medical and psychosocial sequelae of course of illness and impact on social functioning (may be specific to patient population); applies biophysical and cultural concepts in the development of the psychosocial plan when consulting with hospital, personnel and other agencies.
  • Analytical ability to assess patient needs at a micro and macro level, develop associated treatment modalities at a more complex level and provide effective counseling.
  • Identifies and analyzes individual and system issues; develops and recommends corrective action; understands and uses principles of negotiation and is successful in achieving outcomes.
  • Proficiency with an electronic patient record, email, general office and communications equipment.
  • Excellent oral and written communication skills necessary to communicate/exchange and interact effectively and professionally with hospital staff, agency health care professionals, patients and families.

Required Licensure, Certification, On-going Training

  • LCSW or LCSW-C Required
  • Board Approved Clinical Supervisor, Maryland Board of Social Work Examiners
  • Specialty certification preferred i.e. OSW-C, ACM-SW, APSWH-C

Salary Range: Minimum 41.09 per hour - Maximum 67.84 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
