Job summary

Prime Home Developmental Disabilities Services, Inc. seeks a Lead Direct Support Professional (DSP) for the Connections program to plan, coordinate, and oversee daily activities at the center and in the community. You will lead staff, support individuals with disabilities, and ensure quality care with proper documentation and reporting.

The role emphasizes leadership, teamwork, and a commitment to creating meaningful engagement and positive outcomes for those served.

Qualifications

Responsibilities

• Develop and maintain a monthly activity calendar.
• Collaborate with coordinators to address questions from individuals and DSP staff.
• Review and assist with data collection, documentation, and billing processes.
• Ensure the facility is clean, organized, and welcoming.
• Plan and implement a consistent schedule of engaging on-site activities.
• Promote active participation and engagement throughout each individual’s day.
• Maintain agency vehicles, ensuring they are clean, fueled, and in proper working order.
• Monitor and maintain appropriate staff-to-individual ratios during community outings.
