Summary

Ensures that all services are delivered in accordance with the Mission Statement and S.P.I.R.I.T. Values of LifeBridge Health. Provides professional, comprehensive, family-oriented services to individuals in their choice of residence. Assesses patients' and families' psychosocial, environmental, and financial needs. Formulates, implements, and evaluates a plan of care in collaboration with patient, family, and interdisciplinary team members. Functions as a consultant, educator and expert practitioner. Serves as organizational resource to ensure evidenced based practice and to facilitate optimal patient/family care. Is accountable for clinical practice issues within area of expertise.

Pay Range: $34.18 to $51.27 Based On Experience
