LifeBridge Health seeks a seasoned social worker to deliver family-centered care in alignment with its Mission and SPIRIT Values. You will assess psychosocial, environmental, and financial needs and collaborate with patients, families and the interdisciplinary team to formulate, implement, and evaluate care plans.

Provides professional, comprehensive, family-oriented services to individuals in their choice of residence. Assesses patients' and families' psychosocial, environmental, and financial needs. Formulates, implements, and evaluates a plan of care in collaboration with patient, family, and interdisciplinary team members. Functions as a consultant, educator and expert practitioner. Serves as organizational resource to ensure evidenced based practice and to facilitate optimal patient/family care. Is accountable for clinical practice issues within area of expertise.
