Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.

REMOTE-FULL TIME-DAYS

Qualifications

  • Bachelor’s degree or equivalent Required.
  • Master's degree or equivalent Preferred

  • LIC-License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire Required Or
  • CRT-Registered Dietitian (RD) – CDR Commission on Dietetic Registration Upon Hire Required Or
  • LIC-Master Social Worker (MSW-Master) - STATE_MI State of Michigan Upon Hire Required Or
  • LIC-Physical Therapist - STATE_MI State of Michigan Upon Hire Required Or
  • CRT-Occupational Therapist, registered (OTR) - NB-COT National Board for Certification in Occupational Therapy Upon Hire Required Or
