Job summary

Rochester Regional Health is seeking a Clinical Leader of Long-Term Care at Clifton Springs Nursing Home to supervise the Long Term Care unit, lead staff rounds with physicians, and ensure high-quality geriatric care. The role requires strong leadership, empathy for patients, and effective communication to maintain staffing and care plans.

Responsibilities include coaching staff, recognizing performance, and coordinating with the Nurse Manager to address staffing needs and high-risk resident

Qualifications

• Two (2) years of RN experience preferred.
• Experience with geriatric population preferred.
• Compassionate, kind and patient focused.
• Demonstrated leadership and communication abilities.

Responsibilities

• Unit & Staff Management: Round on staff and physicians; resolve issues; complete rounding log; identify high risk residents and ensure care plans are current; ensure adequate staffing.
• Communication & Evaluation: Recognize staff performance; communicate staffing needs to Nurse Manager; coach staff privately when needed.
