The Lead Clinical Case Management Supervisor will provide case management oversight of the referral and intake process of the SIL program, providing leadership and training to case‑management staff. The role establishes effective relationships with people with mental health issues and meets the goals of individual client‑centered care plans, which may include service coordination, medication management support, assistance with life skills training, finding and maintaining housing and healthcare. A collaborative and results‑oriented leader who will supervise master’s level social work and multidisciplinary case‑management staff and manage overall operations of an integrated services program. The supervisor monitors outcomes toward achieving program and individual outcomes, coordinates with other agency departments, hires and evaluates staff, develops resources and implements training to ensure a well‑trained, high‑functioning team, and provides direct oversight of all program aspects to ensure quality and compliance. The Clinical Supervisor reports to the Clinical Program Director.
Typical work is performed in an office or community‑based setting with standard lighting, temperature and noise levels. The position requires periods of sitting, standing and walking throughout the day. Employees may need to move between offices, program spaces and meeting areas, including stairs. Occasionally the supervisor must lift, carry or move items up to 15 pounds. The role requires operating office equipment, talking and hearing in person and by telephone, and reading printed and digital materials. Regular travel to multiple agency and partner locations within the Bay Area is required; reliable transportation is essential.
