LiveWell Homecare Agency is seeking a Registered Nurse to deliver high-quality in-home nursing care by assessing, planning, implementing, and evaluating patient needs as part of a collaborative care team.

You will serve as a liaison between patients, families and healthcare providers, ensuring safety and patient-centered care, and you will participate in professional development and leadership activities while maintaining licensure.

Qualificações

• Associate or Bachelor’s degree in Nursing is preferred.
• Current RN license; multi-state compact license preferred.
• BLS certification required upon hire.
• Strong communication, teamwork, and critical thinking skills.
• Commitment to professional development and quality patient care.

Responsabilidades

• Monitor and evaluate patient conditions, assist in care planning, and implement appropriate interventions.
• Communicate care plans effectively with team members and prioritize patient flow for optimal outcomes.
• Deliver safe, effective care, document assessments, and report changes or incidents per policies.
• Address patient and family concerns, provide education, and promote satisfaction through compassionate advocacy.
• Contribute to performance improvement efforts, suggesting solutions and supporting organizational goals.
• Maintain licensure and certifications, pursue professional growth, and engage in leadership and mentorship activities.
• Adhere to confidentiality, safety standards, and professional conduct, promoting a positive work environment.
