Job summary

Cantex Continuing Care Network seeks a Charge Nurse (LVN/LPN) to provide direct patient care and nursing leadership in a long-term care setting. You will oversee care delivery, coordinate care plans, and ensure compliance with physician orders and licensure requirements.

You will supervise CNAs, promote patient safety, and support staff performance while maintaining accurate documentation and efficient operations in a caring environment.

Qualifications

• Graduate of an accredited Nursing School with a current LVN/LPN nursing license in the applicable state.
• Minimum of one year supervisory experience in a long-term care or healthcare setting preferred.
• Ability to read, write, and speak English.

Responsibilities

• Observe, assess, document, and report patient conditions and changes.
• Maintain accurate EMR documentation related to patient assessments and nursing care activities.
• Consult with Registered Nurses and Nurse Managers regarding changes in patient condition and clinical needs.
• Receive, transcribe, and execute physician orders using approved methods and company procedures.
• Implement care plans and administer medications and treatments as prescribed.
• Assign duties to CNAs and monitor delivery of patient care services.
• Ensure incidents are documented and reported per policies and standards.
• Coordinate availability and maintenance of equipment and supplies used in patient care.
• Promote a safe environment and support abuse prevention requirements.
• Perform duties as assigned.
