• The N-MCM conducts ongoing needs assessments, monitors care plans, and supports patients toward self-sufficiency and viral suppression.

General Description:

The Non-Medical Case Managercoordinatespsychosocial support services forpeopleliving with HIV/AIDS. The case managerprovidesa range of client centeredactivities focused on improving access to and retention inneeded core medical and support servicesthe N-MCM provides coordination guidance andassistancein accessing medicalsocial community legal financial employment vocation and/or other neededservices.The N-MCM provides case management services at all agencylocations,offsite locations within othercommunity-basedpartnerorganizations,andcurbside/home visits when needed. The N-MCM conducts ongoing needs assessmentmonitors ofcareplans documentsreferring tooutcomes to supportpatientstowardsself-sufficiency. Thecase management goalforeach patient is to obtain viral suppression through amulti-teamapproach.
