- Do you have experience providing supervision hours in an acute care hospital setting?
JOB DESCRIPTION:Lead Care Manager LCSW at our Health Center, located in Santa Monica, CA.NOTE: The position available is for Full Time, 12:00pm-8:30pm shift.- Utilize your transferrable skills to assess and coordinate care for a diverse population of patients in the community.
- Collaborate with our Multidisciplinary Team as well as patients and their families to ensure safe, high quality, and effective delivery of care.
- Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community.
- The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management.
- Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).
REQUIRED QUALIFICATIONS:- Master's Degree in Social Work from an accredited program.
- Upon hire: California Clinical Social Worker License
- 2 years of experience in healthcare related field (Acute, Ambulatory, or Post-Acute)
- 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
