Job Description

• Lead Care Manager LCSW at our Health Center, located in Santa Monica, CA. The position available is for Full Time, 12:00pm-8:30pm shift.

• Utilize your transferrable skills to assess and coordinate care for a diverse population of patients in the community. Collaborate with our Multidisciplinary Team as well as patients and their families to ensure safe, high quality, and effective delivery of care. Join our Care Management Team at our hospital!
• Case Management is a collaborative practice including patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. The Case Management process facilitates communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

Required qualifications:

• Master's Degree in Social Work from an accredited program.
• Upon hire: California Clinical Social Worker License
• 2 years of experience in healthcare related field (Acute, Ambulatory, or Post-Acute)
• 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
