Job summary

The Enhanced Care Management (ECM) Lead Care Manager at Mercy House coordinates essential medical and social supports for clients experiencing homelessness, addressing SDOH needs across settings. You will collaborate with CalAIM Community Supports staff to deliver patient-centered, team-based care and removal of barriers to service access.

You will maintain a caseload, engage clients in services, coordinate care, and document progress while working with a multidisciplinary team in a cooperative,

Qualifications

• Bachelor’s Degree in Social Work (or related field) with experience in chemical health and mental health preferred.
• Strong outreach and engagement skills for people experiencing homelessness.
• Experience with care coordination and case management for homeless populations.
• Spanish language ability is a plus.
• Excellent documentation and data-entry skills.
• Ability to work in a fast-paced team environment.

Responsibilities

• Maintain a caseload of 17–25 individuals and complete Health Needs Assessments and Care Plans.
• Engage, outreach, and enroll ECM members in services and address barriers.
• Provide services where the member lives or seeks care within CalOptima Health guidelines.
• Coordinate medication reviews, appointments, reminders, and transportation when needed.
• Attend team meetings and collaborate with hospitals and providers.
• Document all client information accurately and timely.
