On Lok is a not-for-profit organization founded in the early 1970s by citizens concerned about the plight of elders and the lack of long-term options in the community. It serves as the administrative arm of five affiliated non-profit organizations and performs research and development, human resources, fundraising, and management information systems.
POSITION SUMMARY
The Lead Care Manager 1 is responsible for coordinating health care resources, providing education, and facilitating access to care for a panel of members. This clinical advocate works as an active interdisciplinary team member and liaison with other health and social service providers. The role may involve managing members with mental health conditions and substance use disorders.
ROLES AND RESPONSIBILITIES
MEMBER PANEL MANAGEMENT & GROWTH
- Manages an assigned panel of members and maintains privacy of member personal and health information.
- Conducts initial and routine home visits and assessments to develop, implement, and update individual care plans and interventions, and makes appropriate timely referrals.
- Performs a minimum weekly on‑site visit to On Lok facilities based on program or operational requirements.
- Performs a minimum weekly in‑person outreach visit based on member’s care plan.
- Outreaches to new candidates and actively participates in program growth.
CARE PLAN DEVELOPMENT & MANAGEMENT
- Identifies comorbidities, hospitalizations, prescriptions, and other factors that could impact member health.
- Identifies care plan goals and objectives in collaboration with members.
- Oversees and supports implementation of the member’s care plan, connecting with members via phone or in‑person to facilitate engagement, follow‑up, and education/training.
RESOURCE & CARE COORDINATION
- Acts as a liaison and advocate between members, their physicians, outside healthcare and social service agencies, and other community providers.
- Collaborates with healthcare providers to help members meet their goals.
- Connects members to other social services and supports they may need.
DEVELOP MEMBER RELATIONSHIPS (PATIENT ENGAGEMENT) & HEALTHCARE RECORDS MANAGEMENT
- Builds trust with members, families, and caregivers by providing support and care.
- Communicates with members about their care plan and engages other care team partners when necessary.
- Accurately documents all member interactions, care plans, and relevant information in the electronic health record (EHR) system in a timely and compliant manner.
- Generates member reports as needed.
ADDITIONAL OPERATING RESPONSIBILITIES
- Participates in and contributes to required team meetings, aggregate report meetings, and trainings for professional development and effective care coordination.
- Performs other duties as assigned.
QUALIFICATIONS
- Bachelor’s degree in Social Work, Behavioral Health, or Public Health.
- Three years of experience in case management and/or a social service setting.
- Minimum one year working with high‑risk or vulnerable populations, serving a frail or low‑income population.
- Knowledge and understanding of HIPAA regulations.
- Experience facilitating positive behavioral changes in members.
- Excellent oral and written communication skills, including proficiency in motivational interviewing and active listening; able to provide consultations via phone, email, video, and in‑person as needed.
- Strong computer literacy and working knowledge of MS Office, with ability to learn new software programs.
- Ability to operate with minimal supervision, demonstrating initiative and independence while working remotely and in the field.
- Strong problem‑solving and teamwork skills in a multidisciplinary environment.
- Critical thinking, good judgment in challenging situations, self‑motivation, and resourcefulness.

Salary: $40.26 - $53.68 per hour
