Homelessness is the largest social and public health crisis in California. Illumination Health + Home is a growing non-profit organization dedicated to disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services, and children's and family programs. IHH currently has 13+ facilities with 22+ micro-communities across Orange County, Los Angeles County, and the Inland Empire.

The Lead Care Manager (LCM) is a community-based, client-facing role where populations are actively engaged, assessed, enrolled, and supported. This person serves as the main contact for clients, building strong relationships to promote engagement with medical care, behavioral health, and social services. The role requires a holistic, non-clinical approach, applying evidence-based practices, understanding service barriers and social determinants of health, and coordinating appropriate services. The LCM helps clients navigate healthcare systems, promotes preventative care, and collaborates closely with the care team.

The pay range for this position is $23-26 per hour, depending on experience.

The schedule for this role is 8:00 am - 4:30 pm.

Client Care Responsibilities

• Serve as the primary contact for clients
• Conduct routine holistic health assessments and monitor changes during hospitalization
• Create and update SMART care plan goals and interventions
• Visit clients’ homes to promote health education and preventative care initiatives
• Accompany clients to medical appointments as needed
• Communicate with care team members to facilitate client care
• Observe, report, and assess medication self-administration
• Connect clients to social services, including transportation
• Coordinate discharge plans with hospital staff
• Manage a caseload of approximately 45 clients, with adjustments as directed by senior management
• Identify service gaps and barriers, document in care plans, and communicate with the multidisciplinary team
• Participate in supervision meetings and provide weekly progress reports

Documentation Responsibilities

• Document all client engagement and activities accurately in Illumination Health + Home’s EMR system
• Record care plan goals, engagement levels, client records, and disengagement details
• Stay informed about services offered by other providers
• Maintain confidentiality and organize client information, schedules, and files
• Track attendance at medical appointments and outreach activities

Qualifications

Required:

• High School Diploma or equivalent
• At least 2 years of relevant experience
• Basic computer skills, including email and data summarization in spreadsheets

Preferred:

• Medical Assistant Certification
• Social Worker Certification
• Community Health Worker Certification
• B.A. in a related field
• Bilingual abilities
