Homelessness is the largest social and public health crisis in California. Illumination Health + Home is a growing non-profit organization dedicated to disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services, and children's and family programs. IHH currently has 13+ facilities with 22+ micro-communities across Orange County, Los Angeles County, and the Inland Empire.
The Lead Care Manager (LCM) is a community-based, client-facing role where populations are actively engaged, assessed, enrolled, and supported. This person serves as the main contact for clients, building strong relationships to promote engagement with medical care, behavioral health, and social services. The role requires a holistic, non-clinical approach, applying evidence-based practices, understanding service barriers and social determinants of health, and coordinating appropriate services. The LCM helps clients navigate healthcare systems, promotes preventative care, and collaborates closely with the care team.
The pay range for this position is $23-26 per hour, depending on experience.
The schedule for this role is 8:00 am - 4:30 pm.
