Job summary

Wisconsin Community Services, Inc. seeks a Lead Care Coordinator to support the REACH-Crisis program and youth/family care coordination in Milwaukee.

You will lead Wraparound processes, mentor new coordinators, and coordinate care plans with essential court and team meetings. You will use Avatar for documentation, manage a ~6 youth caseload, and collaborate with Milwaukee County Behavioral Health Services to improve service quality.

Qualifications

• Bachelor's degree in a relevant field required.
• Minimum of one year of mental health services experience, with youth/adolescent focus preferred.
• Lead Care Coordinators must have at least one year of Wraparound Milwaukee care coordination experience.

Responsibilities

• Provide leadership and role modeling of the Wraparound process for Care Coordinators.
• Mentor and train new care coordinators; provide problem-solving feedback to staff.
• Conduct hands-on community training for the first 6 months.
• Attend Team/Plan of Care Meetings and Court Hearings to coach staff.
