Position & Service Specifications

• Oversee six Transplant Hepatology APP direct reports - all in the outpatient setting.
• Practice alongside five Hepatologists
• Mon-Fri, 8-hour shift schedule
• Report directly to the Transplant Service Line Chief Administrative Officer
• Inpatient and outpatient responsibilities
• Work closely with divisions including Transplant Surgery, Hepatology, Nephrology, Critical Care, Hospital Medicine, Infectious Disease, Interventional Radiology, Anesthesiology, Nutrition, Pharmacy, Case Management, and Rehabilitation Services
• Opportunity to teach and mentor Transplant Hepatology residents, fellows, and students in an academic setting

Duties & Responsibilities

• Onboard and mentor new Transplant APPs
• Participate in recruitment and interviews for new APP positions
• Manage PTO and scheduling needs
• Conduct performance evaluations
• Communicate operational, programmatic, and institutional updates to the APP team
• Lead quality improvement initiatives within the Transplant program
• Maintain consistently availability for providers to cover calls, labs and medications
• Provide comprehensive care for patients with chronic liver disease, including those undergoing liver transplant evaluation and those who are actively listed for transplantation.
• Collaborate with hepatologists, transplant surgeons, and the multidisciplinary team to optimize medical management of pre-transplant patients.
• Travel to 1-2 outreach clinics per month in the Chicago and Northwest Indiana area to deliver high-quality liver care and transplant evaluation services, improving access for patients in the community.
• Perform comprehensive assessments, formulate diagnoses, and initiate and manage treatment plans
• Manage immunosuppressive therapy, identify and treat complications, and coordinate with multidisciplinary teams
• Document care in the EMR (Epic) with accuracy and timeliness
• Participate in multidisciplinary selection meetings, transplant quality reviews, and protocol development
• Engage in clinical research, process improvement initiatives, and program development as desired
• Contribute to education of medical students, APPs, and residents through case discussions and clinical mentorship
• Coordinate care at discharge (home health, PT/OT, medication reconciliation, clinic follow up)
• Input pre- and post-transplant orders for medications and diagnostic studies
• Participate in weekly meetings pertaining to transplant candidate selection along with quality meetings

Preferred Qualifications

• Candidates must have a minimum of three years of Advanced Practice Provider experience in Hepatology, Abdominal Transplant or related specialty.
• APRN Candidates may require Acute Care Certification.

Position Requirements

Nurse Practitioner

• Current licensure from the State of Illinois as a Registered Professional Nurse and Advanced Practice Nurse or eligible to obtain a licensure through the Illinois Department of Financial and Professional Regulation (IDFPR) application process
• Master of Science in Nursing or Doctorate of Nursing practice with clinical specialty appropriate to position
• Current certification by American Nurse Credentialing Center (ANCC) or American Academy of Nurse Practitioner (AANP)
• CPR and ACLS certifications

Physician Assistant

• Current licensure from the State of Illinois as a Physician Assistant/Associate or eligible to obtain a licensure through the Illinois Department of Financial and Professional Regulation (IDFPR) application process
• Successful completion of an accredited Physician Assistant/Associate Program
• Current certification from National Commission on Certification of Physician Assistants
• CPR and ACLS certifications

The total pay range shown reflects the wide range of factors that are considered in making compensation decisions including, but not limited to, knowledge and skills; relevant experience and training; education, certifications, and licensure; primary work location; and other business and organizational factors. This total pay range is for a full-time, 1.0 FTE employee, and includes any incentive payments that may be applicable to this role, as well as additional compensation such as sign-on and retention bonuses and relocation allowances.
