Job summary

Highbridge Healthcare is seeking a Licensed Clinical Social Worker (LCSW) in Connecticut to join our multidisciplinary team in Stamford. You will provide individual and group therapy to residents, applying social work principles to develop personalized treatment plans that support recovery and quality of life.

Collaborating with physicians, nurses, and therapists, you will monitor progress, adjust plans, and offer crisis intervention as needed.

Qualifications

• CT-licensed LCSW with current CT license.
• MSW or related field required.
• Minimum 2 years in a Long-Term Care Facility.
• Excellent communication and interpersonal skills.
• Ability to work independently and in a team.

Responsibilities

• Provide individual and group therapy for patients with mental health concerns.
• Develop and implement individualized treatment plans.
• Assess progress and adjust treatment plans as needed.
• Collaborate with physicians, nurses, and therapists for comprehensive care.
• Provide crisis intervention and emergency services as needed.
• Maintain accurate records of sessions and plans.
• Participate in quality improvement initiatives.
