Skilled Healthcare Center in Stamford, CT seeks a Licensed Clinical Social Worker (LCSW) to join our multidisciplinary team. You will provide individual and group therapy, develop personalized treatment plans, and coordinate with physicians, nurses, and therapists to ensure high-quality care.

The ideal candidate holds CT LCSW credentials, an MSW or related degree, and at least 2 years of experience in a Long-Term Care Facility.

Competenze

Must hold CT Licensed Clinical Social Worker (LCSW) license.
Master's degree in social work (MSW) or related field.
Minimum 2 years of experience in a Long-Term Care Facility.
Strong understanding of social work principles and practices.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.

Mansioni

Conduct individual and group therapy sessions with patients, addressing anxiety, depression, and trauma.
Develop and implement individualized treatment plans based on patient goals and progress.
Assess patient progress and adjust treatment plans as needed.
Collaborate with physicians, nurses, and therapists to ensure comprehensive care.
Provide crisis intervention and emergency services as needed.
Maintain accurate and timely records of patient sessions and treatment plans.
Participate in quality improvement initiatives to enhance patient care and outcomes.
