Rutherford Regional Health System is seeking a Social Worker (LCSW) to join a mission-driven team dedicated to making communities healthier. You will interview, assess, coordinate services, and assist with discharge planning to ensure comprehensive patient care and smooth transitions.

The role requires a Master’s degree, Michigan LCSW license, BLS-HCP certification, and at least 2 years of related experience. A full benefits package accompanies the position.

Qualifications

• Master’s degree required.
• Michigan LCSW license or equivalent state license.
• BLS-HCP certification; Crisis Intervention Training within 90 days of hire.
• Minimum 2 years related experience.

Responsabilités

• Interviews and assesses patients and/or family, caregivers and legal representatives.
• Determines, prioritizes, provides and/or arranges for needed services/interventions.
• Participates in case reviews to evaluate case management and progress; collaborates with the healthcare team.
• Assists with discharge planning and identifies resources for the patient’s needs.
• Performs Medicaid screenings as indicated.
• Assists patients with Safety Net applications when needed.
