Advocate Health in Charlotte, NC seeks a Social Worker to provide biopsychosocial assessments, diagnosis, and treatment planning within a cardiology/pediatrics context. You will interview patients and families, deliver brief psychotherapy, and coordinate care with a multidisciplinary team to improve outcomes.

The role requires a CSWE-accredited MSW, a Clinical Social Work license, and ability to mentor junior staff. Full-time, 40 hours/week with competitive benefits are offered.

Qualifications

• Master's Degree in Social Work from a CSWE approved graduate school.
• 1 year of experience in current department or 2 years in healthcare.
• License to practice Clinical Social Work in the applicable state and/or SC.
• Proof of membership in a professional organization.
• Ability to mentor/supervise new social workers as needed.

Responsabilités

• Provides biopsychosocial assessments, diagnoses, and treatment planning.
• Interviews patient and social network for psychosocial evaluation.
• Provides brief psychotherapy and supportive counseling.
• Coordinates care for patient/family with multidisciplinary team.
• Develops and revises plan of care based on needs.
• Delivers patient/family education and monitors outcomes.
• Advocates for patient/family across care continuum.
• Maintains accurate documentation per policy.
• Mentors provisionally licensed social workers and staff.
