We are currently recruiting for a LCSW or LPC or LMSW to join our school-based team at an elementary school in Bridgeport. This position is 35 hours per week and year-round.

POSITION SUMMARY Our School-based behavioral health clinicians provide direct services to Optimus’ patients and families with the goal of developing patient independence and self-sufficiency, when applicable. The school-based program provides services to students who attend schools that our clinicians are imbedded in. We provide treatment for patients who are diagnosed with a mental illness or refer to appropriate service provider. We oversee, administer, coordinate and/or assist in other designated Optimus programs and functions as assigned.

Essential Functions & Responsibilities

  • Provides clinical behavioral health services and case management, information & referral services to individuals and families. This includes biopsychosocial assessments and psychotherapy, treatment planning, collaboration with and referral to other members of the health care team internal and external.
  • Maintains appropriate documentation.
  • Responsible for the accurate and complete preparation of encounter information including appropriate coding for diagnosis and procedure; DSM-5 and CPT Coding to ensure prompt billing for services rendered.
  • Generates initial treatment plans and updates every three (3) months. Discharge summaries are generated within thirty (30) days after patient transferred, discharged or patient terminated treatment.
  • May provide supervision, consultation, and oversight for some of the designated OHC programs and staff. This includes the ongoing and changing grant-funded programs at OHC and the clinical and non-clinical staff. These responsibilities also include conducting performance evaluations, in-service training, task assignments, and supervision of staff, which include goal planning and career development.
  • Assist in program development as directed by the Director of Behavioral Health and / or Administration.

PROGRAM COORDINATION

  • Coordinate and implement assigned program as required per funding source
  • Collect monthly data from all participants to ensure participation in the designated program, activity level and patient needs are met.
  • Conduct regular assessment of curriculum to ensure program is functioning as implemented.
  • Provide refresher (booster) training as needed on how to apply the program to be able to extract required reporting data.
  • Prepare and submit report to funding source as required (monthly and/or annually)
  • Present at annual conferences as needed.

  • Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment.
  • Uses tactful, appropriate communications in sensitive and emotional situations.
  • Follow up as appropriate with supervisor and co-workers regarding reported complaints, problems, and concerns.
  • Promotes positive public relations with patients, family members, and guests.

Job Qualifications/Requirements

EDUCATION: LMSW/LCSW: Master's degree in social work from an accredited university or LPC-A/LPC: Master's degree in area of specialty and at least 2 years' experience in the field of mental health counseling or other related fields and Undergraduate degree in counseling, psychology, sociology or an undergraduate degree in counseling, psychology, sociology, or social work

LICENSURE / CERTIFICATION: Licensed under the State of Connecticut Department of Public Health. A valid driver's license is also required.

EXPERIENCE: A minimum of two (2) years of related experience. Some behavioral health experience is preferred in the areas of culturally diverse populations. Combination of clinical, crisis intervention, systems, case management, conflict resolution, behavior modification, program development, child development, state and local resources, and group skills needed. Good verbal and written communication skills are required. Basic computer skills are desired. Familiarity with telemedicine and prior experience are a plus.

LANGUAGE SKILLS: Spanish language skills are preferred.
