UF Health in Gainesville, FL is seeking a Clinical Social Worker to provide and coordinate psychosocial and behavioral services for a defined patient population. You will collaborate with the care team to assess, plan, implement, and monitor a care plan, supporting safe transitions from hospital or clinic.

The role involves crisis intervention, identifying abuse/neglect, and delivering direct services and resources to patients as needed.

Formación

• MSW degree from a Council on Social Work Education accredited program.
• Recent clinical experience applicable to the designated population served.
• Experience in healthcare setting preferred.

Responsabilidades

• Provide and coordinate psychosocial and behavioral services for the assigned patient group.
• Collaborate with physicians, nurses, case managers, PT/OT staff, pharmacists to plan and implement care.
• Assist with discharge planning and transition from hospital or clinic.
• Provide crisis intervention and assess for potential abuse/neglect of vulnerable patients.
