UF Health in Gainesville, FL is seeking a Clinical Social Worker to provide and coordinate psychosocial and behavioral services for a dedicated patient group. You will assess, plan, implement, and monitor care in collaboration with physicians, nurses, case managers, PT/OT staff, and pharmacists to ensure safe transitions from hospital or clinic.

The role emphasizes crisis intervention, advocacy, and direct services to patients, including resources for discharge planning.

Formación

• MSW degree from CSWE-accredited program.
• Recent clinical experience relevant to the population served.
• Preferred experience in a healthcare setting.

Responsabilidades

• Provide and coordinate psychosocial and behavioral services for the patient group.
• Assess, plan, implement, and monitor the care plan in collaboration with the medical team.
• Provide crisis intervention and assess for abuse/neglect as needed.
• Advocate for patients and assist with discharge planning and transition from hospital or clinic.
