Job summary

Western CT Home Care Inc seeks a Medical Social Worker to provide clinical social work services in home health, collaborating with physicians and care teams to develop and update individualized plans of care for patients and their families.

Responsibilities include counseling patients and families, documenting goals and progress, participating in case conferences, coordinating referrals to community resources, and ensuring compliance and ongoing education.

Qualifications

• Experience in a qualified home health agency.
• Minimum three years of relevant social work experience.
• LCSW credential preferred or required.

Responsibilities

• Provides services ordered by the physician as per the plan of care.
• Counsels patients, caregivers and families; provides education.
• Addresses social and emotional factors affecting health problems.
• Participates in case conferences with the health care team and others as needed.
• Identifies needs for other home health services and referrals.
• Refers patients and families to appropriate community resources.
• Records goals and progress notes for physician review.
• Participates in quality assessment and improvement activities.
• Completes required continuing education as scheduled.
• Maintains compliance with all responsibilities attached to the role.
• Demonstrates reliable attendance and professional conduct.
• Maintains the values of the agency in daily practice.
