Adoration Health in Griffin, GA is seeking a licensed clinical social worker (LCSW) for home health services. The PRN role involves weekly availability, providing patient-centered social work support to Henry, Pike & Spalding coverage areas.

You will guide individuals and families through crisis, assist with government funds, and develop care plans to improve patient outcomes in a home health setting.

Qualifications

• Master’s degree in Social Work.
• Five or more years of clinical social work experience.
• Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license.
• Ability to perform psycho-social assessments and develop viable care plans.

Responsibilities

• Guide and counsel individuals and families in crisis.
• Assist clients in obtaining government funds, education, or treatment.
• Secure information on medical, psychological, and social factors and evaluate them.
• Help patients and families through individual or group conferences regarding plans for meeting needs.
• Help the client modify attitudes and patterns of behavior to improve social functioning.
• Develop a service plan designed to restore the patient to optimum social and health adjustment.

Responsibilities

• Guide and counsel individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
• Assist clients in obtaining government funds, education, or treatment
• Secure information, such as medical, psychological, and social factors contributing to the client's situation, and evaluate these and the client's capacities. Ask clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments, to determine the nature and degree of the problem
• Help patients and families through individual or group conferences regarding plans for meeting needs and aid clients in mobilizing inner capacities and environmental resources to improve social functioning
• Help the client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and the client's part in creating them
• Develop a service plan designed to restore the patient to optimum social and health adjustment within the patient's capacity
