Adoration Health is seeking a Social Worker to join our Eastman, GA home health team. You will provide patient-centered social work services, crisis counseling, and care planning to support patients and families in the Dodge, Wilcox, Telfair, Wheeler counties area.

This PRN role offers flexible scheduling, independent work with team support, and opportunities for dedicated advancement within a growing organization. Requires a Master’s in Social Work and LCSW/LMSW licensure.

Qualifications

• Master’s degree in Social Work required.
• Five or more years of clinical social work experience.
• Valid and current Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) license.
• Ability to perform psycho-social assessments and develop viable care plans.

Responsibilities

• Guide and counsel individuals and families in crisis.
• Assist clients in obtaining government funds, education, or treatment.
• Secure and evaluate medical, psychological, and social information to determine problems.
• Lead individual or group conferences to plan needs and mobilize resources.
• Help clients modify attitudes and behavior by increasing self-understanding.
• Develop service plans to restore patients to optimal social and health adjustment.

Responsibilities

• Guide and counsel individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
• Assist clients in obtaining government funds, education, or treatment
• Secure information, such as medical, psychological, and social factors contributing to the client's situation, and evaluate these and the client's capacities. Ask clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments, to determine the nature and degree of the problem
• Help patients and families through individual or group conferences regarding plans for meeting needs and helps clients in mobilizing inner capacities and environmental resources to improve social functioning
• Help the client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and the client's part in creating them
• Develop a service plan designed to restore the patient to optimum social and health adjustment within the patient's capacity

Qualifications

• Master’s Degree in Social Work
• Five plus years of clinical social work experience
• Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
• Ability to make psycho-social assessments and develop and implement viable care plans
