Collage Home and Community Rehabilitation Partners in Houston, TX, is seeking experienced Licensed Clinical Social Workers to provide truly patient-centered care in patients' homes and communities. Part-time positions are available for clinicians who will accept patients for the duration of their stay.

Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities.

Qualifications

• Master's degree in Social Work and state licensure as an LCSW are required.
• Excellent planning, organization, writing, and time management skills.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsibilities

• Assess, counsel, and direct medically complex patients.
• Develop individualized treatment plans to assist adjustment to disability and behavior modification.
• Coordinate resource linkage and support patients in their homes and communities.
