Job summary

ComoCare Stamford, CT is seeking a Licensed Clinical Social Worker (LCSW) to deliver high-quality individual and group therapy within our intensive outpatient program. You will collaborate with the care team to ensure treatment plans support lasting improvement and recovery, while refining the clinical curriculum as the program evolves.

Ideal candidates have 4–5 years of outpatient behavioral health experience, trauma-informed care, and strong documentation skills.

Qualifications

• Licensed Clinical Social Worker (LCSW) certification/license
• 4–5 years of outpatient or community-based behavioral health experience
• Experience facilitating group and individual counseling
• Experience collaborating with interdisciplinary care teams
• Trauma-informed care approach
• Strong communication and documentation skills

Responsibilities

• Provide high-quality individual and group therapy within the intensive outpatient program (IOP)
• Deliver patient-centered care tailored to each individual's behavioral health needs
• Guide patients from intake through completion, supporting engagement and retention
• Monitor progress and outcomes, maintaining accurate clinical documentation
• Collaborate with care team to coordinate holistic patient support
• Contribute to ongoing refinement of the clinical curriculum and care delivery
• Coordinate with FQHC care teams to ensure appropriate care and resources
• Track KPIs including clinical quality, retention, completion, and patient satisfaction
