Job summary

Nashville General Hospital is seeking a Licensed Clinical Social Worker for the Patient Centered Medical Home in Nashville, Tennessee. This full-time role operates on a Day Shift within a hospital setting.

The LCSW will conduct psychosocial assessments, provide counseling, coordinate care with medical staff, and connect patients with community resources while maintaining compliant documentation and participating in team huddles and ongoing training.

Qualifications

• Experience working with adults with mental, behavioral, or emotional disorders.
• Ability to assess personal and external factors to plan care.

Responsibilities

• Coordinate clinical updates with medical providers to ensure appropriate social work interventions.
• Provide psychosocial assessment and treatment under supervision as needed.
• Plan referrals and coordination with health and social services for optimal patient care.
• Conduct individual counseling for patients in crisis or per physician referral.
• Develop and implement care plans acceptable to patient, family and team.
• Engage in care planning, timely referrals, and daily care team huddles.
• Train clinic staff on mental health topics including confidentiality and substance use.
• Educate patients and families about care plans and community resources.
• Maintain documentation in the electronic health record per standards.
• Support continuity of care across clinic and external agencies.
• Participate in QA activities and in program development.
