Job summary

MedStar Union Memorial Hospital seeks an Inpatient Case Manager to oversee care from pre-admission through post-discharge, coordinating patient plans and resources. You will work with multidisciplinary teams to ensure cost-effective, high-quality care and develop comprehensive discharge plans tailored to individual outcomes.

The role requires a Master’s in social work, hospital experience, and licensure (LCSW-C/LICSW). CCM is preferred, reflecting a clinical focus in Maryland or DC locations.

Qualifications

• Master’s degree in social work from a CSWE-accredited school.
• 1–2 years’ experience in social work, preferably in a hospital setting.
• LCSW-C or LICSW license required/depending on work location.
• CCM – Certified Case Manager preferred.

Responsibilities

• Develop a care plan addressing continuum of care with timelines and family involvement.
• Coordinate referrals to community agencies and initiate timely referrals.
• Facilitate communication between patient, family, and healthcare team; participate in rounds.
• Complete psychosocial history or socioeconomic assessment as required.
• Develop and maintain information on community resources and referral requirements.

Job Summary

MedStar Health is currently seeking an Inpatient Case Managerto join our team atMedStarUnion MemorialHospital.Thispositionfacilitates the delivery of quality, cost effective, patient-centered care from pre-admission through post-discharge timeframe.

As a member of the Case Management team, you will monitor the care and services delivered to selected patient populations during the acute hospital stay, promoting effective case management and utilization of resources facilitating the continuum of care to achieve optimal clinical outcomes. Additionally, you will collaborate with multidisciplinary health care teams to develop and coordinate plans of care designed to meet individualized patient outcomes.

Primary Duties

• Demonstrate the ability to develop a plan of care that addresses needs across the continuum; develop long and short-term goals with specific time frames for resolution; identify specific services to be provided in the care plan; include the family/caregiver in the plan of care; and show life planning contingencies such as power of attorney and/or advance directives.
• Collaborate with and complete referrals toappropriate communityagencies forassistancebased on patientneed. Initiate referralsin a timely manner. Utilize resources to helppatientresume life in the community and/or to adjust to lifestyle changes.
• Facilitate communication between the patient and family and the healthcare team. Assess and communicate the patient's social, cultural, emotional, and economic problems to medical staff and other healthcare providers as needed. Participate in multidisciplinary rounds and collaborate with internal and external healthcare providers,patientand family to develop a comprehensive discharge plan, as necessary.
• Complete psychosocial history or socioeconomic assessmentin a timely mannerper policy standards when referred by healthcare team or asdeterminedby high-risk indicators.
• Develop andmaintaininformation on community resources and referral requirements based on patient population asidentifiedin theunitscope of practice.

Qualifications

• Master’s degree in social work from a school accredited by the Council on Social Work education
• 1 – 2 years’ experience in social work, preferable in a hospital setting
• 1 – 2 years’ experience in care/case management preferred
• LCSW-C - Licensed Certified Social Worker – Clinical License in the District of Columbia or the State of Maryland depending on work location OR LICSW - Licensed Independent Clinical Social Worker License in the District of Columbia or the State of Maryland depending on the work location
• CCM – Certified Case Manager preferred

This position has a hiring range of

USD $74,214.00 - USD $134,596.00 /Yr.
