Job summary

AMN Healthcare seeks an LCSW to provide home health clinical social work services in a fast-paced patient-centered environment. The role requires licensure and involves coordinating with an interdisciplinary team to support patient discharge planning and psychosocial assessments.

Qualifications

• BLS (AHA) certification required.

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

Monday-Friday 08-4:30pm. We have our weekly Interdisciplinary team meeting on Thursday mornings that is a requirement to attend.
