Job summary

LiveWell Homecare Agency, LLC is seeking a registered nurse to provide direct patient care in a hospital setting. The role focuses on patient- and family-centered care, illness management education, and adherence to safety and regulatory standards.

The nurse will develop individualized care plans, collaborate with care teams, and address physical, psychosocial, and emotional patient needs while honoring cultural preferences.

Qualifications

• Nursing Diploma or Associate’s Degree required; Bachelor’s Degree preferred.
• RN license required.
• No experience required; 2+ years preferred.
• Strong assessment, communication and critical thinking abilities; collaborative teamwork.

Responsibilities

• Provide direct patient care including assessment, diagnosis, planning, implementation and evaluation.
• Educate patients and families on illness management, safety and post-treatment care.
• Develop and document personalized care plans with clear goals and communicate across care teams.
• Work in a safety-sensitive environment with alertness and adherence to practices.
• Participate in safety, quality and compliance activities and ongoing professional development.
