Job summary

Amae Health in Los Angeles is seeking a driven Clinical Outreach Manager to own referral growth. This hybrid role combines territory-building business development with hands-on patient advocacy.

You will build and manage a network of hospital systems, EDs, discharge planners, and outpatient providers to create a steady stream of qualified referrals, and guide patients through warm handoffs to care in hospital, at home, or in the community.

Qualifications

• 3+ years in a quota-carrying, relationship-driven role in healthcare or B2B sales.
• Proven track record building or growing a territory or book of business.
• Existing familiarity with the Los Angeles healthcare landscape is a strong plus.
• Comfort with targets and data-driven goals.
• Willingness to be in the field daily across the LA region.

Responsibilities

• Referral Partner Development & Territory Growth: build and manage referral partners in hospitals, EDs, community mental health centers, and outpatient providers in LA; run recurring outreach; activate new sources; build relationships at multiple levels; re-engage dormant sources; represent Amae Health at events and meetings; be the face of Amae Health in the market.
• Warm Handoff & Patient-Facing Support: manage warm handoffs by meeting patients in hospital, home, or community; coordinate with hospital systems to move patients into care; guide patients through the process.
• Pipeline Management & Reporting: maintain CRM records; own quarterly market plan; report weekly on pipeline health and risks.
