Location: Los Angeles & Orange County Expected Hours: 15 – 30 per week Pay: $40-45 per visit plus mileage

Job Description:

As a Hospice Certified Home Health Aide (CHHA), you will provide compassionate, personalized care to patients in their homes, following the Hospice Aide Plan of Care. Your role is essential in ensuring the comfort, dignity, and quality of life of our patients during their end-of-life journey.

Key Responsibilities:

Personal Care: Provide personal care services such as bathing, dressing, toileting, and feeding according to the Hospice Aide Plan of Care.

Documentation: Accurately document the care provided in accordance with agency policy.

Reporting: Immediately report any deviations from the Plan of Care to the Case Manager or their designee.

Equipment Use: Safely operate and maintain Durable Medical Equipment.

Vital Signs: Monitor and record vital signs as indicated on the Hospice Aide Plan of Care.

Team Collaboration: Foster a cooperative environment by understanding the roles of other team members and actively participating in team and staff conferences.

Skin Care: Provide proper skin care and observation to prevent tissue breakdown, especially over bony prominences, and report any concerns to the Case Manager.

Mobility Assistance: Assist with ambulation, transfers, and range of motion exercises as documented in the Hospice Aide Plan of Care.

Family Support: Offer respite care for the patient’s family or caregiver, as appropriate.

Quality Improvement: Participate in Quality Assurance and Performance Improvement (QAPI) activities.

Interdisciplinary Meetings: Engage in Interdisciplinary Group (IDG) meetings as appropriate.

Job Conditions:

Physical Requirements: Ability to perform physical assessments and patient care, which may include walking, sitting, stooping, standing, and lifting or turning patients.

Company Information:

Confidentiality: Will have access to patient medical records.

Qualifications:

Training: Must meet state training requirements, including a minimum of 75 hours of classroom and supervised practical training, with at least 16 hours each of classroom instruction and supervised practical training.

Experience: At least one year of experience as a CNA, hospice aide, or Hospice aide (hospice experience preferred).

Age: Must be at least 18 years old.

Licensure: Must be a licensed driver with an insured automobile in good working order.

Communication: Strong verbal and written communication skills are required.

Certifications: Must hold a Certified Home Health Aide (CHHA) certification.

Medical Specialty:

Hospice & Palliative Medicine

Application Requirements:

Experience: Minimum of 1 year of hospice experience required.

Language: Korean language proficiency preferred.

Licenses/Certifications: Must possess a valid Driver's License and Certified Home Health Aide (CHHA) certification.

Location: Ability to commute to Buena Park, CA.

Travel: Willingness to travel 100% of the time.
