Job summary

Community Health Resources (CHR) seeks a Kinship Navigator for the Connecticut Kinship Navigator Program in Manchester. This full-time, 40 hours/week role offers Mon-Fri hours with a flexible schedule around caseload, no weekends.

The Navigator conducts needs assessments, connects families with community resources, advocates for family needs, and collaborates with a supportive team. Bilingual Spanish preferred; a driver’s license and vehicle are required for home visits; salary starts at around

Qualifications

• Bachelor’s degree preferred in human services; alternatives considered as described.
• Experience working with families and knowledge of community resources.
• Driver’s license and access to a vehicle for home visits.
• Bilingual Spanish preferred.

Responsibilities

• Conduct needs assessments across basic needs, welfare, education and health.
• Assist families in identifying and prioritizing needs and accessing resources.
• Identify culturally appropriate resources within the community.
• Advocate for family needs to all involved parties.
• Document services and maintain regular supervision.
