You will lead front-line efforts to ensure quality and seamless care, educate patients and families, and support donors and recipients through the pre-transplant process.

Qualifications

• RN license required; other licensure may substitute for RN.
• Bachelor's degree required.
• 10 years transplant coordinator experience may substitute for Bachelor's degree.
• Knowledge of organ procurement.

Responsibilities

• Coordinate transplant program activities with a focus on quality and continuity of care.
• Educate patients, families and healthcare providers on transplant processes and care pathways.
• Assist with donor-recipient matching using the Extended Criteria Donor program.
• Provide pre-transplant support and coordinate on-site care for patients in the program.
