Qualifications

• Bachelor's Degree preferred.
• 2+ years of RN experience required.
• 1 year of acute care disease-related hospital experience required.
• 3 years of RN experience preferred.
• Transplant experience or GI experience preferred.
• ICU experience preferred.
• Endoscopy background preferred.
• Basic Life Support (BLS) Certification required.

Responsibilities

• Assess and manage kidney transplant patient activities with the transplant physician.
• Coordinate plans of care and symptom management with outpatient caregivers.
• Schedule follow-ups with the administrative coordinator.
• Review referring physician information and initiate communication.
• Coordinate multidisciplinary consults and obtain follow-up information.
• Assist in discharge planning for transplant patients and donors.
• Assist in obtaining medications and supplies per insurance coverage.
• Arrange local follow-up or at Cleveland Clinic facilities as needed.
• Collaborate with transplant and referring physicians on candidacy evaluation.
• Assemble evaluation test results and arrange further testing.

What You'll Do

• Assess patients in the outpatient clinic along with the transplant physician.
• Facilitate plans of care and symptom management with outpatient caregivers.
• Coordinate scheduling with the administrative coordinator for follow-up appointments.
• Review medical information from referring physicians and initiate appropriate communication.
• Coordinate consultation with other disciplines and obtain follow-up information.
• Coordinate with hospital unit personnel in the discharge of transplant patients and donors.
• Assist in obtaining medications and supplies according to the patient's insurance coverage.
• Contact referring physicians and arrange follow-up in the local area or at CCF.
• Coordinate with the transplant physician and referring physician in evaluating the patient for transplant candidacy.
• Assemble test results of evaluation and arrange further testing if indicated.
