St. Elizabeth Physicians in Kentucky is seeking a Float Advanced Practice Provider to join our Primary Care Division. The AP Provider will float between offices to cover patient panels, out-of-office coverage, and leave coverage, working 40 hours per week.

The Advanced Practice Provider will assess, diagnose, prescribe, treat and educate patients, collaborating closely with physicians and clinicians to ensure continuity of care and high-quality outcomes for diverse patient populations.

Qualifications

  • APRN or PA license and current state licensure.
  • Minimum 2 years clinic experience plus 1 year mid-level experience.
  • Ability to obtain KY and/or IN license for scope of practice.

Responsibilities

The Advanced Practice Provider is primarily responsible for providing patient care services, including assessing, diagnosing, prescribing, treating, and educating patients. The Advanced Practice Provider works closely with physicians and other clinicians in a team approach to patient care. The Advanced Practice Provider is a highly visible position that is always responsible for creating a positive impression with patients, physicians and other visitors he/she encounters, both in person and on the phone.

Knowledge of advanced practice including anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities. Knowledge of medical equipment and instruments used to administer patient care. Knowledge of common safety hazards and precautions to establish/maintain a safe work environment. Knowledge of health promotions, health risk identification, and patient education principles and techniques. Knowledge of community, professional and education resources. You must be able to obtain a state license in KY and/or IN for your scope of practice

Duties And Responsibilities

  • Assesses patient status by obtaining health history records info in EMR.
  • Assess presenting illness, risk factors and family history.
  • Assess psychosocial situation and cultural factors.
  • Performs appropriate physical examination.
  • Orders/performs appropriate laboratory, diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians and other clinicians.
  • Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.
  • Develops and implements treatment plan by prescribing/dispensing medication and/or injections in compliance with medical practice guidelines and state laws.
  • Instructs patient/family regarding medications and treatments. Educates patient regarding health promotion/illness prevention. Recommends appropriate community resources to meet patient/family needs.
  • Prepares documentation for medical records including updating patient medical chart and EMR by documenting examination, test results, diagnosis, medication and treatment in written/computerized manner.
  • Participates in peer reviews, EMR reviews, employee education, clinical guideline development and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies and procedures. Complies with patient confidentiality requirements and promotes patient advocacy.
  • Collaborates with physicians in managing acute and long-term medical needs of the patient. Provides monitoring and continuity of care between visits according to treatment plan including triaging patient calls and messages.
  • Other duties and responsibilities as assigned are complete on a timely, thorough and accurate basis.

Required Skills And Knowledge

Licenses And Certifications

Current State license as an Advanced Practice Registered Nurse or Physician Assistant.

Education

Minimum: APRN or PA degree/certification required.

Years Of Experience

2+ years of experience in a clinic setting plus 1 year of mid-level experience.
