Job Summary

The Care Partner supports the care team under the RN's direction, promoting a positive patient experience, assisting with daily living activities, and performing basic patient care functions such as ADL's, ambulation, and vital signs. Advanced skills like phlebotomy, dressing changes, and Foley catheter discontinuation may be required based on unit needs.

Core Responsibilities and Functions
  • Partner with healthcare team to ensure excellence in patient experience.
  • Provide patient-centered care, supporting wellness and addressing patient needs.
  • Maintain confidentiality and adhere to HIPAA regulations.
  • Participate in hourly patient rounding, answer call lights promptly, and address patient concerns.
  • Assist with direct patient care, including hygiene, nutrition, and comfort measures.
  • Support team initiatives, transport supplies and medications, and support performance improvement efforts.
  • Communicate effectively using established techniques, document care accurately, and maintain equipment.
  • Follow safety and infection control policies, including proper hand hygiene and environment cleanliness.
Minimum Education and Certification Requirements
  • Rising senior or GED; certification as a Nurse Assistant preferred.
  • CPR/BLS certification required upon hire; Nurse Assistant certification preferred.
  • Additional certifications like phlebotomy or EKG are a plus.
Skills and Experience
  • Ability to follow detailed instructions and communicate effectively.
  • Strong interpersonal skills and ability to work independently and in a team.
  • Minimum age of 16 years.
