Job summary

Human Services Administration Organization (HSAO) is seeking a Juvenile Justice Service Coordinator in Pittsburgh, PA. This full-time role involves coordinating services for youth involved with the juvenile justice system, partnering with families, schools, and treatment providers to reduce barriers and improve outcomes.

The coordinator will complete assessments, develop service and crisis plans, make referrals, and connect clients with community resources while collaborating with the Juvenile

Qualifications

  • Bachelor’s degree in a relevant field (Master’s degree welcome).
  • Two years of experience in behavioral health, juvenile justice, or human services setting.
  • Strong organizational skills with the ability to manage multiple priorities and meet deadlines.
  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a multidisciplinary team.
  • Demonstrated cultural competence and the ability to work with diverse populations.

Responsibilities

  • Serve as the single point of accountability for clients and families, acting as the primary contact and advocate for services.
  • Build and maintain enduring relationships with clients and their families through outreach and engagement.
  • Develop comprehensive service plans tailored to individual strengths and needs, regularly updating and presenting progress to the court.
  • Support clients during transitions, including return to the community from treatment programs, detention facilities, and hospitalization.
  • Coordinate care across providers including behavioral health, juvenile probation, child welfare, and schools.
  • Maintain monthly face-to-face contact with clients and document interactions in HIPAA-compliant records.

Position Overview:

Juvenile Forensic Service Coordinators provide specialized service coordination to 25-30youth(ages 10 – 18) involved with Juvenile Justice System. Service Coordinators complete assessments, individualized service plan and crisis plan, treatment referrals, and connections to community resources to address social determinants of health. Staff closely collaborate with Juvenile Justice System to support clients with meeting the expectations of the court. Successful service coordinators build trusting relationships with youth and families, maintain timely documentation, meet program utilization, coordinate multidisciplinary care, and achieve service plan goals.

Key Responsibilities:
Client Support and Case Management
  • Serve as theSingle Point of Accountabilityfor clients and families, acting as the primary contact and advocate for services.
  • Build andmaintainenduring relationshipswith clientsand their familiesthrough persistent outreach and engagement.
  • Developcomprehensive service planstailored to individual strengths and needs, regularly updatingandpresentingprogressto thecourt.
  • Support clients duringtimes of transition, including those preparing for return to community from treatment programs, detention facilities, and hospitalization.
  • Coordinate careacross various providers, including behavioral health,juvenile probation,child welfare,drug and alcohol services,schools, and community resources.
  • Maintainmonthly face-to-face contactwith clients, ensuring services are provided in line with program standards.Meet withclients throughout Allegheny County, including homes, schools, treatment programs,courts, juvenile probation placements, detention centers, county jail, etc.
Collaboration and Advocacy
  • Work closely with families,mental health services,probation officers, educational staff, and community providers to addressbarrierstoservice plan goals.
  • Facilitateinterdisciplinary team meetings, ensuringfollow-through onaction stepsand ensuring continuity of care.
  • Act as anadvocatefor clients, helping remove systemic barriers,resolving issues, and fostering full inclusion within their community.
  • Attend school-based meetings and offereducational advocacyas needed, ensuring clients have the support they need to thrive.
Documentation and Compliance
  • Completeassessments, service plans, and case noteswithin specifiedtimeframes, ensuring all documentationcomplies withstate and agency regulations.
  • Maintainaccuraterecordsof all interactions, including family meetings, service linkages, and follow-up activities, in electronic health records.
  • Meetmonthlyutilizationexpectations.
  • Comply withHIPAA confidentiality policiesandparticipatein regular audits and program evaluations.
Professional Development
  • Participate inongoing trainingand development opportunities, staying updated on best practices and trends inbehavioralhealth, juvenile justice,education,and service coordination.
  • Activelyutilizeindividual and group supervisionto triage clients,monitortrends, enhance the program.
Required Skills and Qualifications:
