Job summary

Catholic Charities seeks a Wayne County Case Manager to collaborate with therapists and support youth in juvenile court programs. You will participate in Strive mentoring and YES activities, delivering community-based services and family outreach.

You will monitor youth wellbeing, assist with treatment planning, and provide crisis intervention as needed, aiming to improve outcomes and reinforce healthy skills.

• Provide in-home support to youth and families as needed.
• Assist with mental health, substance use and trauma screenings.
