Position Summary

Unified Care Hospice is seeking a dedicated and compassionate Medical Social Worker to join our team. The Medical Social Worker provides support to patients and their families to ensure they receive the best possible care during the hospice journey. The successful candidate will have a strong background in social work, excellent communication skills, and a commitment to the highest standards of care.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and their families to identify their unique needs and challenges.
• Develop and implement individualized care plans, in coordination with the interdisciplinary hospice team, to address emotional, social, and practical aspects of care.
• Provide emotional support, counseling, and education to patients and families to help them cope with end-of-life issues.
• Advocate for patients and assist in connecting them with community resources, financial assistance, and other support services.
• Collaborate with the interdisciplinary team, including physicians, nurses, chaplains, and other professionals, to provide holistic care to patients.
• Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions.
• Participate in regular team meetings to discuss patient care and provide input on care plans.
