Job summary

Legacy Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our nursing team for full-time hospice care across Clarke, Frederick, Shenandoah, and Warren County. Schedule is Monday–Friday, 8:00 am–4:30 pm with rotating call, delivering hands-on care and coordinating services for patients and families.

The role emphasizes compassionate, high-quality end-of-life care as part of an interdisciplinary team, with a strong focus on education, advocacy, and supporting families

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical-surgical/acute care setting; hospice experience a plus.
• Must be computer proficient, with EMR experience.
• Current CPR certification if state requires it.
• Valid driver's license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility for services.
• Support patients and families to ensure comfort and quality of life.
• Deliver nursing care rooted in compassion and clinical excellence.
• Educate patients and families to participate in personalized care plans.
• Share on-call responsibilities as part of the team.

Legacy Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm and Rotating Call

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$87,000 - $92,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
