Job summary

Legacy Hospice is seeking Per Diem Licensed Practical Nurses to provide direct patient care in the Norfolk area under RN supervision. Responsibilities include following the plan of care, administering treatments, and collaborating with the care team to achieve positive patient outcomes.

Ideal candidates will have an LPN license, 3+ years of LPN experience in community health or acute care, and CPR certification if required. Hospice/home health experience is preferred.

Qualifications

  • Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
  • Holds current unencumbered LPN license in state of practice.
  • Minimum three years' LPN experience in community health or acute care.

Responsibilities

  • Assess and evaluate patient status per plan of care and update as needed with RN Case Manager.
  • Provide focused physical assessment considering physical, emotional, and spiritual factors.
  • Attend and support at time of death as required by physician orders and local regs.
  • Maintain communication with attending physician and RN Case Manager; relay changes in patient condition.
  • Maintain timely, accurate electronic medical records documenting actions, goals, and changes.

Legacy Hospice is currently hiring Per Diem Compassionate Licensed Practical Nurses (LPN) to provide patient care in and around the Norfolk Area!

Our Licensed Practical Nurses (LPNs) are responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include, but are not limited to, following the plan of care, providing treatment, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Territory: Newport News and Norfolk

Responsibilities
  • Assesses and evaluates patient's status by following the plan of care, regularly reevaluating patient and family/caregiver needs, and participates in revising the plan of care as necessary based on patient status and needs in conjunction with the RN Case Manager.
  • Provides a focused physical assessment while being aware of the physical, emotional and spiritual aspects which may influence health, well-being and quality of life.
  • Attends and provides support at the time of death (with physician's order) and per local regulations.
  • Maintains regular communication with the attending physician and RN Case Manager regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
  • Monitors and maintains documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness via-electronic record so that problems, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
Qualifications
  • Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
  • Holds current unencumbered license in the state of practice as an Licensed Practical Nurse.
  • Minimum three (3) years' experience as LPN in Community Health, Medical surgical/Acute care setting required.
  • Hospice/home health experience preferred but not required.
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.

$30.00 - $33.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
