Guardian Angel Senior Services, Inc. is hiring an ISCH Worker to provide individual support to Massachusetts Waiver Contracted Individuals.

You will help clients maximize independence, develop daily living skills, and build community connections while maintaining a safe home environment. Join a team that collaborates with caregivers, PCAs, and interdisciplinary staff to promote health, safety, and overall well-being.

Qualifications

• Previous home care or human services experience
• Knowledge of community support resources

Responsibilities

• Support individuals in making informed decisions regarding their care, health, and goals
• Assist with the development and implementation of care plans
• Encourage independence, community participation, and socialization
• Work collaboratively with interdisciplinary care teams
• Provide resources and support to promote safety and independent living
• Assist with caregiver and PCA training and supervision
• Conduct home visits and accurately document services provided
• Maintain timely and accurate client documentation
• Communicate scheduling changes, concerns, and client needs with office staff
• Maintain professional and therapeutic relationships with clients
• Protect client privacy and maintain strict confidentiality
• Recognize and report concerns regarding abuse, neglect, or safety
• Attend required orientation, staff meetings, supervision, and training
• Follow all agency policies and applicable state and federal regulations
• Maintain regular access to email for scheduling and agency communications
• Transport clients when appropriate using a clean, insured, and properly maintained vehicle
• Work within each individual's service plan and established goals
• Perform other duties appropriate to the position

Individual Support to Massachusetts Waiver Contracted Individuals

Guardian Angel Senior Services is looking for compassionate, reliable, and dedicated professionals to join our team as an ISCH Worker.

ABOUT THE ROLE

The ISCH Worker provides regular or intermittent support to Massachusetts Waiver Contracted Individuals. The position focuses on helping individuals:

• Maintain and maximize independence
• Develop self-care and daily living skills
• Improve socialization and communication skills
• Build confidence and community connections
• Maintain a safe and supportive living environment
• Establish and maintain residence within the community
• Achieve goals outlined in their Individual Service Plan

You will work closely with individuals, caregivers, PCAs, agency staff, and interdisciplinary teams to promote the individual's health, safety, independence, and overall well-being.
