Job summary

Job description

Scope / Position Summary

This position functions to coordinate all aspects of patients’ pharmaceutical needs in the patient care areas. The duties include but are not limited to:

• Assurance of appropriate dose, route, frequency and duration of therapy
• Patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens
• Coordination of patients’ pharmaceutical needs with physicians and nurses
• Supervision of pharmacy residents and students, medical residents and students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy)

Patient population includes adult, adolescent, and geriatric patients.

Minimum Qualifications

Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred. Ability to perform point-of-care INR.
