Job Summary

We are currently looking for a Registered Nurse licensed in Wisconsin to become our next IRIS Self-Directed Personal Care (SDPC) RN. This is a remote position, where you will partner with people in your community who are enrolled in the Wisconsin IRIS program – a Medicaid long-term care option for older adults and people with disabilities. People in the IRIS program who need personal care services have the choice to enroll in the IRIS Self-Directed Personal Care (IRIS SPDC) option. You can learn more about IRIS SDPC on the Wisconsin Department of Health Services website here, and learn about the IRIS program here. While this role is home-based, you will have regularly scheduled visits with people in their homes and communities.

Responsibilities
  1. Provide oversight and guidance to IRIS SDPC participants, including assessments, oversight, training, and education.
  2. Administer the Wisconsin Personal Care Screening Tool and create person-centered plans of care.
  3. Ensure care plans are up-to-date, obtain physician orders, and conduct oversight visits and calls.
  4. Submit Prior Authorization requests for personal care services.
  5. Comply with all Department of Health Services policies, SDPC guidelines, procedures, and documentation requirements.
  6. Provide training and educational materials to participants and care providers.
  7. Coordinate with IRIS Consultants, Long-Term Care Functional Screeners, and physicians to meet care needs.
  8. Complete documentation and follow-up as required.
  9. Perform other duties as assigned; overtime and travel may be required, including driving up to 50% of the time during home visits.
Qualifications
Required Education

Associates Degree in Nursing

Required Experience
  • Minimum 2 years of nursing experience, including at least 1 year in home health serving individuals with disabilities or the elderly.
  • Proficiency with Microsoft Office Suite, database management, and data entry.
  • Excellent communication, teaching, mentoring, and problem-solving skills.
  • Strong organizational skills and ability to work independently.
  • Flexibility to adapt to organizational changes.
Licenses and Certifications

Current unrestricted RN license in Wisconsin, valid Driver’s License.

Preferred Education and Experience
  • Bachelor’s Degree in Nursing.
  • Experience with Wisconsin Medicaid Personal Care Program or home care experience.
Additional Details
